Patient Applications

Instructions for Applying
● Download both the Patient Application and the Checklist.
● Completely fill the Patient Application.
● Gather the documents listed in the checklist.
● Submit both the Completed Application and the Documents from the Checklist to Project Access for review.
● After you submit your completed application packet, please call Project Access to check the status of your application.

Instrucciones Para Aplicar
● Descargar la Aplicacion para el Paciente y la lista de Verificacion de Requisitos.
● Completar toda la Aplicacion para el Paciente (4 Paginas).
● Recaudar los documentos establecidos en la lista de Verificacion de Requisitos.
● Enviar la Aplicacion completa junto con los documentos establecidos en la lista de Verificacion de requisitos para ser evaluados por Project Access.
● Después de someter la aplicación completa junto con los documentos necesarios, por favor llame a Projecto Access 205-202-1598 para checar el estatus de su Aplicación .
